The following SQL leads to an assertion fault:

CREATE TABLE t1(x INTEGER PRIMARY KEY UNIQUE, b) WITHOUT ROWID;
CREATE INDEX t1x ON t1(x);

This problem appears in all versions of SQLite that support WITHOUT ROWID tables, which is to say all versions since 3.8.2 (2013-12-06).

This problem was discovered by OSSFuzz.
